The Palo Alto Medical Foundation (PAMF) and its Research Institute have received a $1.2 million federal grant from the Agency for Healthcare Research and Quality to study the use of personal health records and remote patient monitoring in managing diabetes - one of the most costly and prevalent chronic health conditions.
